A Family Care Plan is the documented plan that ensures a Soldier’s dependents will be cared for when the Soldier is deployed or otherwise unavailable. The official form used to create and submit that plan is DA Form 5305, which is specifically titled Family Care Plan. It provides the information about who will provide care, how the plan will be carried out, and how family and unit contacts will coordinate. The other forms are used for different purposes (identification/deers enrollment, medical records release, or unrelated duties), so they aren’t the form used to document a Family Care Plan.
